US Family Health Plan currently owns and operates three health centers in New York: the Bay Ridge Family Health Center in Brooklyn, the Mitchel Field Family Health Center in Long Island, and the Fort Wadsworth Family Health Center in Staten Island.

Our centers proudly provide primary care services to USFHP members. The offices are staffed by board certified physicians, and knowledgeable and friendly clinical and administrative support staff.
