 The US Family Health Plan is a specially legislated TRICARE option within the military health system. US Family Health Plan offers TRICARE Prime, a managed care program that provides comprehensive coverage at the lowest cost to members of the three TRICARE options. - Member choice: You choose a participating US Family Health Plan Primary Care Physician (PCP) who gets to know you and helps you to navigate the complex world of health care. Your PCP sees you for annual physicals, provides care when you're sick or injured, and coordinates your care with specialists.
Comprehensive health care: US Family Health Plan covers PCP and specialists visits, hospitalization, prescriptions, and medical emergencies anywhere. USFHP extra services such as annual physicals and eye exams. Furthermore, US Family Health Plan carefully selects PCPs and specialists who meet high standards of medical practice.
- Low cost: For all members, there are no deductibles and no exclusions for pre-existing conditions and there is a limit to how much you would pay in the event of a catastrophic illness. In addition, for dependents of active duty personnel and retirees with Medicare Part B, there are no enrollment fees and no co-payments (except for prescriptions).
- Hassle-free: There are no claim forms to fill out and US Family Health Plan can assist you in the coordination of benefits.
- Part of the military family: US Family Health Plan's roots are in the Military Health System; we are not a commercial insurance company. The US Family Health Plan is administered by Saint Vincent Catholic Medical Centers, which has a respected history for over 20 years.

| Enrollment is easy - you can sign up any time throughout the year. Simply download and print out the Member Enrollment application and return it to us.
